Persons enrolled in the Saitama City National Health Insurance program and are aged 40 to 74.
*Persons who have not paid their national health insurance tax are not eligible to receive this medical examination.
*Persons who have received the Specific Medical Checkup (特定健康診査) are not eligible to receive this Comprehensive Medical Examination.
From end of April to March of each fiscal year.
* You may only receive the examination once during this period.
* For more information on the dates of the medical examination, contact the National Health Insurance and Pension Division in your respective ward.
A list of eligible medical institutes and doctors and a Specific Medical Examination Ticket (特定健康診査受診券) will be sent to you in April. You can also see the list in Japanese here.
* Reservation is required so please make an appointment before you go.
* If you did not receive or have lost the medical examination ticket, contact the National Health Insurance and Pension Division in your respective ward.

Comprehensive Medical Examination Form * This form is required to receive the Comprehensive Medical Examination. * To receive this form, you must first make a reservation at an eligible medical institute and then bring your Specific Medical Examination Ticket and national health card with you to the National Health Insurance and Pension Division in your respective ward. |

* Those who receive the Comprehensive Medical Examination are not eligible to receive the Saitama City cancer checkup (for stomach cancer, lung cancer and colon cancer).
* Base on your medical examination results, you may be asked to receive health guidance from the doctor.
