Eligibility:

Persons enrolled in the Saitama City National Health Insurance program and are aged 40 to 74.

• Persons who have not paid their national health insurance tax are not eligible to receive this medical examination.
• Persons who have received the Specific Medical Checkup (特定健康診査) are not eligible to receive this Comprehensive Medical Examination.

When?

From end of April to March of each fiscal year.

• You may only receive the examination once during this period.
• For more information on the dates of the medical examination, contact the National Health Insurance and Pension Division in your respective ward.

Where?

A list of eligible medical institutes and doctors and a Specific Medical Examination Ticket (特定健康診査受診券) will be sent to you in April. You can also see the list here.

• Reservation is required so please make an appointment before you go.
• If you did not receive or have lost the medical examination ticket, contact the National Health Insurance and Pension Division in your respective ward.

Things to bring:

・Comprehensive Medical Examination Form (see below)
・Specific Medical Examination Ticket
・National health insurance card 

Cost:

About 30,000 yen.
* Tests not covered by the National Health Insurance comprehensive medical examination will incur additional fees.

What’s included in the Specific Medical Examination?

·consultation with physician,
·physical measurement (height, weight, etc.),
·blood pressure measurement,
·blood lipid test,
·liver function test,
·blood sugar test,
·urinalysis,
·renal function test,
·electrocardiogram test,
·anemia test
·pancreatic function test,
·serological verification,
·respiratory examination,
·digestive examination, and others.

* Those who receive the Comprehensive Medical Examination are not eligible to receive the Saitama City cancer checkup (for stomach cancer, lung cancer and colon cancer).
* Base on your medical examination results, you may be asked to receive health guidance from the doctor.
