Persons enrolled in the Saitama City National Health Insurance program and are aged 40 to 74.
From end of April to March of each fiscal year.
A list of eligible medical institutes and doctors and a Specific Medical Examination Ticket (特定健康診査受診券) will be sent to you in April.

1.Specific Medical Examination Ticket
【What is the Specific Medical Examination Ticket?】
At the end of April each year, a Specific Medical Examination Ticket —which is required to receive the Nobinobi Health Checkup (Specific Health Checkup)— will be sent to your home.
If you do not receive the ticket or if you lose it, please contact the Health and Pension Division at your local ward office.
2.National health insurance card
Documents that confirm your enrollment in Saitama City National Health InsurancePlease present one of the following:
Free of charge.
*Only if necessary: eye exam
