School Admission Health Examination

To enter elementary school, your child must receive a health checkup.
Under Japanese law, this health checkup is required.
Around early September each year, children who will start elementary school the following April will receive a Notice of School Entry Health Checkup (a letter asking you to take your child for a health checkup).
Once you receive this notice, please be sure to have your child take the health checkup.

What to bring to the health checkup

1.  Notice of School Entry Health Checkup
2.  Health Checkup Form ※ Please fill out the parts for the parent/guardian and bring it.
3.  Health Checkup Questionnaire ※ Please complete and bring it.
4.  Maternal and Child Health Handbook
5.  Something to write with (pencil or pen)
6.  Handkerchief and tissue ※ Please give your child a handkerchief.
7.  Indoor shoes (uwabaki) ※ Slippers and sandals are not allowed.
8.  Plastic bag (to put outdoor shoes in)

Flow and Content of the Health Checkup

1.  Submit your documents and check in
2.  Your child will have exams for: - Eyes, ears, nose, teeth - Internal medicine - Development/intelligence check
3.  The results will be explained
4.  DoneNotes:

*   The order of tests may be different depending on the school.
*   Depending on the test, the exams may not follow the order of check-in.
*   In some cases, the parent/guardian may need to stay with the child during the checkup.
*   For vision or hearing tests, your child might not be used to group testing, and the result may seem poor. 
→ If that happens, please visit a hospital or consult someone.

Things to Be Careful About When Taking the Health Checkup

  • A person who knows the child’s usual condition and health should come together with the child.
  • On the morning of the health checkup, take the child’s temperature. Also take the temperature of the person accompanying the child.
  • If the child or the accompanying person has a fever, cough, or feels unwell, do not come for the checkup. At reception, the health condition of both will be checked.
  • Be sure to follow the reception time. Do not arrive more than 30 minutes early.
  • Do not come by car.
  • If there is bad weather (like a typhoon or heavy rain), the health checkup may be canceled. Check the Saitama City website to confirm.
  • During the checkup, guardians may be asked to wait in the waiting room or stay with the child.
  • The end time of the checkup varies by school. Please contact the school directly.
  • Dress your child in clothes they can put on and take off by themselves. ※ Clothes like one-piece outfits are not allowed.
  • Please write the child’s name on their clothing.
  • For the ear exam, if the child has long hair that covers the ears, tie it so the ears are clearly visible.
  • For the dental exam, make sure the child brushes their teeth before coming.

Q&A on School Admission Health Examination

Q1: Is the school for the health checkup the same one you will enter in April?
A1: Yes.

Q2: If you move out of Saitama City before the checkup day, what should you do?
A2: Please have your child take the health checkup at the new school in your new city instead of Saitama.For details, contact the Board of Education in your new municipality.

Q3: If you move out of Saitama City after the checkup day, what should you do?
A3: You still need to have the health checkup in Saitama.After completing the moving-out procedures, contact the school that had your checkup in Saitama.Submit the checkup documents to the Board of Education or school in your new municipality.

Q4: If you will move within Saitama City—can you have the checkup at the new school?
A4:  If you move after the checkup day, have the checkup at the designated school, not the new one.
  If you move before the checkup day, you can take the checkup at the new school.

Q5: If the checkup day conflicts with another appointment,what should you do?
A5: Please rearrange the other appointment and attend the health checkup.

Q6: For those entering private or national schools, do you still need the checkup?
A6: Yes, have the checkup at the designated school.Once your entrance to the private or national school is confirmed, inform the school where you had the checkup.If school admission is confirmed before the checkup day, check with the school whether you still need to attend the Saitama City checkup.If you cannot attend, be sure to contact the designated school by the checkup day.

Q7: What if you plan to enter a special needs school or class?
A7: If you have any worries or need special support during the checkup, please consult with the Special Needs Education Counseling Center staff.

Q8: What time will the checkup end?
A8: The finish time varies by school.Please ask the school where you will take the checkup directly.

Q9: If I need assistance during the checkup, what should I do?
A9: Contact the school where you are going to take the health checkup as early as possible to request assistance.
